Appointments

We see all patients on an appointment basis and ask you
to call in advance so that we may schedule time for you.
To schedule an appointment, please call Donna
at
762-5696 during our normal office
hours. Be ready to provide the following information:

Whether you are a new or established patient
of Dr. Poland's.

A succinct description of any problems
you are having with your eyes.

Medical insurance coverage, daytime
phone number, etc.

Donna, Lou Anne, Vickie or another staff member
will likely ask you a series of questions to determine the urgency and
proper exam slot for you.
We respect your time and make every effort to remain on
schedule. Please bear in mind that emergencies and unexpected
events do occur. If you are unable to keep your appointment,
please notify us as soon as possible. We would appreciate at
least a 24 hours notice if you must change an appointment.

We encourage you to call with questions
you may have concerning your eyes; however, it would be
unfair to other patients if Dr. Poland was to disrupt
treatment to answer every phone call. Our staff is highly
trained and can answer most commonly asked questions. If
your call requires Dr. Poland to speak with you, our staff will
take a message and Dr. Poland or a staff member will
return your call at the earliest opportunity. You may

In Case of Emergency
-
In the event that an acid or a base (i.e. lye) gets into
your eyes, immediately flush the eye with water for at
least 30 minutes. Then report to the emergency room of Via Christi Regional Medical Center.

In the event that an object penetrates the eye, DO NOT
REMOVE IT! Protect the eye and try to prevent movement and
immediately go to the emergency
room of Via Christ Regional Medical Center.

If an urgent situation arises while our office is
open, call the office and you will be screened to
determine the best course of action. If the office is
closed, you may call the switch-board operator at
765-3321 and she will forward your message to Dr. Poland
or you may want to go directly to the emergency room of Via Christi Regional Medical Center.

The Emergency Room of
Via Christi Regional Medical Center is located at the corner
of Hartford and 14th Street. Phone number is (580)
765-3321.

We are located in Doctor's Park on Fairview Avenue. We
are upstairs - park in front and come in the front door
and follow the signs. The is a large number "3" on the
front of our building, above the archway leading into our building.

Exiting 14th Street on Bradley (where
WalGreen's is
located), you would go west to Fairview, and then take a
left (South). We are located in the first large professional building on the right (West) hand side.

Exiting 14 Street at Patton (where Via
Christ Regional Medical Center is
located), you would go west to Fairview and then turn
right (North). We are located on the left (West) hand
side. We are the last large professional building on the
left. There is a "Outpatient Surgery Center"
sign in front of our building.

Insurance
We ask the patient to file
their insurance directly. If you have Medicare, we will file your Medicare claim for you.
Also, if you are a member of Pro-Net, Preferred Community
Choice, or CompMed PPO's, we will file the insurance claim for you. If
you have questions regarding your coverage or how to file
your insurance, please ask Donna.
PPO's: We participate in the
following PPO's: Aetna (i.e., ConocoPhillips), Preferred Community Choice,
Fiserv (i.e., Via Christi Hospital), PPO Oklahoma (i.e., City of Ponca
City, Smith International Cooper Cameron) and CompMed. If you have one of
these insurance coverage, we will file your insurance for you and you
will be responsible for only the applicable co-pay and any
non-covered charges at the time of your exam.
Medicare: We will
file your Medicare claim for you using electronic
submission. The check from Medicare will go directly to
you, as Dr. Poland does not accept Medicare Assignment.
You are responsible for filing
your secondary insurance,
though your claim may automatically roll over to your
secondary insurance.
Filing your Secondary
Insurance to Medicare:
When Medicare sends you their check, they include a form called an
Summary of Benefits.
Copy this form and send it
to your secondary insurance (address on your insurance
card). That's it! All the information your secondary
insurance needs to process your claim is on the Medicare Summary of
Benefits.

New Patients
If you are a new patient to Dr. Poland,
please arrive about 10 minutes before your appointment
time to complete your information sheet. Please bring any
current insurance cards and glasses or contact lenses you
are wearing. Most patients are dilated for their exam, so
you may wish to bring a driver is you are concerned about
driving while dilated and if you have a pair of
sunglasses, please bring them.
The new patient exam includes the
following: an examination of the lids and lashes and the
front and back parts of the eye, being checked for
glaucoma, cataracts, diseases of the eye and a
prescription for glasses.

- Dr. Poland is usually performing cataract surgery Tuesday
mornings.

In an effort to keep down the cost of your medical care,
it is the policy of the Ponca City Eye Clinic to request
payment for services at the time they are rendered.
